FOMMS 2009: Foundations for Innovation

FOMMS attendees wishing to participate in the Workshop on Simulations in Education are asked to indicate their interest by completing this brief form.

Only name and email address are required.

If you wish to be considered for a grant of $500 toward the FOMMS registration fee, please complete the other questions also. If funds prove to be limited, preference will be given to applicants who are attending FOMMS for the first time, or who are otherwise acting to increase the number of registered attendees at the FOMMS 2009 conference.

Note: Anyone receiving grant funds is obligated to use a module in a course in the 2009-10 academic year and assist with assessment activities related to its use. We understand that the information given about teaching plans may reasonably change after this form is submitted, so the details provided there are not binding.
